Sensei Naito performed Heian Nidan in front of the class a few years ago in Edinburgh and his kick looked very much like Kegomi. A kegomi attack to the opponents knees would probably be quite effective. This also ties in with your earlier remark - look for the simple applications in kata.

Don't really know (yet?) where I'm trying to get to with this discussion but I'm thinking about the possible "purpose(s)" of Kata such as :-
1) "Simple" Training Aid devised to incorporate a "reference set" of basic Shotokan Techniques put together with various directional movements and turns and stance changes with the intention of providing Kihon Practice Sequences.
2) "Not So Simple" Library of effective Self Defence techniques/Combos. presented as 1) above but with interpretation/application of techniques allowed.
3) Both of the above......
....so obviously the ability to "vary" the kicking techniques to suit a particular Bunkai/Oyo/Application fits in quite well with 2) .......
